A bulbous tip is usually refined with tip plasty: reshaping and suturing the lower lateral (alar) cartilages to add definition, often with a columellar strut or septal-extension graft for support and projection. Alarplasty is different — it narrows wide nostrils/base and is added only if needed. Korean surgeons frequently use the patient's own septal or ear cartilage for natural, ethnically-appropriate definition rather than over-projecting the tip. Open rhinoplasty (a tiny columellar incision) is common for tip work because it gives precise control, though experienced surgeons can do closed in simpler cases. The goal of a refined-but-natural tip is very achievable with the right cartilage technique.
